Patients may exhibit needs outside of the health care field. A social services specialist is on staff to make community referrals for those that are in need. These referrals include transportation options, help with utilities, food needs, housing needs and other items. The care of the whole person is of great importance at Health Partners.

financial counselors

Health Partners of Western Ohio is not a free clinic. Financial counselors are on staff to provide information on the sliding fee scale offered to patients of the health center. If a family's income is less than 200% of the Federal Poverty Guidelines, their fees will be reduced to make them affordable. Various insurance plans are accepted, including: Medicaid, Medicare and commercial insurance. No patient will be turned away regardless of their ability to pay that day.

Health Partners uses the Ohio Benefit Bank to help patients apply for Medicaid. Staff members help patients complete the application, make copies that are needed and will call Job & Family Services for you.
