At Health Partners, behavioral health counselors work with patients for short periods of time to help find solutions to problems.

Our counselors work with the doctors and nurse practitioners at the health center to identify how medical and behavioral health problems are interacting. All patients are screened for depression, substance abuse, problems with medications that might not be working well together and any other behavioral issues interfering with their health. Our behavioral health providers are Licensed Independent Social Workers. They work with patients to focus on the present issues and how to improve their health.
